There are now new versions available of the forms required to apply for an EHC. These have been released by APHA and are now ready to use. The older versions should be discarded although they have indicated they will accept both versions until 31st August 2016.
These must be accompanied by the latest pdf version of the certificate being applied for and any other supporting documents required.
The new forms are –
EXA10 – pre-certificate application for USA 2016 EXA10 Pre Certificate PDF Application
EXA07 – application form for all other countries 2016 EXA07 Meat, Dairy, Misc PDF Application
ET135 – cancel and replace form if a certificate needs to be replaced or has been lost 2016 ET135 cancel+replace form
